EvalSafetyGap: A Hybrid Survey and Conceptual Framework for LLM Evaluation-Safety Failures

This paper combines a hybrid survey - a systematic search paired with narrative synthesis and separately tracked grey evidence - with a conceptual framework and a structured ten-model audit.
